Lab at Home – Terms and Conditions

1. Manzil Home Health Services LLC hereby confirms that all patient information will be kept confidential and will not be used or disclosed outside the company unless required by law. Only authorized personnel from Manzil Home Health Services LLC will have access to patient identifiable information, and any personal information you provide as part of the service/program.
2. I understand that this service involves the collection of bodily fluids and could involve venepuncture for blood collection
3. I understand a partial or follow-up visit is sometimes necessary.
4. I understand and agree to the following T&Cs;

Sevices Included:

1. Registered Nurse
2. Consumables
3. Transportation of Samples to Laboratory
4. Results delivered via email

 Manzil Health Care Services reserves the right to:

1. Adjust the rate based on the services required by the patient/client and authorized by Manzil
2. Change staff at Manzil’s Sole Discretion
3. Discontinue services if the patient’s or client‘s medical condition or behavior becomes unsustainable within the current services
4. If the patient/client cancels the Service less than twenty-four (24) hours prior to when a service is scheduled, Manzil will charge the patient/client 100% of the fees
